Attention Cotulla ISD Parents & Guardians of students in 7-12 th Athletics and CHS Marching Band:
Cotulla High School and Frank Newman Middle School are NO LONGER USING Dragonfly Max for Athletic/Band Online Forms. Cotulla ISD is only using RANK ONE SPORT to fill out Online Forms Click the Rank One Sport tab to access these forms. You will need your Student’s School ID number and Health Insurance Info ready. A Drug Test must also be completed before competing in extracurricular activities. Forms can be picked up at the school. Drug tests must be done at South Texas Rural Health Services and are FREE for CISD students. You can email or turn in completed Physicals and Medical History forms to the Athletic Trainer.
